If the student athlete signing and submitting the form provided above is a legal minor, then the student athlete hereby declares that they have been granted permission/consent/authorization by their parent(s) or legally assigned guardian(s) to complete and submit the form, along with any other information that has been provided to SOLID KNOCK by the student athlete.
The student athlete further declares that the information provided accurately depicts the student athlete’s views and/or performance indices, as well as certifying that all other information provided is accurate and not intentionally misrepresented.
Finally, the student athlete acknowledges – with the permission of the student athlete’s parents or legally assigned adult guardian -- that SOLID KNOCK is granted permission to use the information provided for the purpose of www.e-SKore.com Beta site’s development and testing; but for no other purpose whatsoever without the additional express, written consent of the student athlete and the student athletes parents or legally assigned adult guardian.
